Academic Misconduct
Academic misconduct violates the principle of intellectual integrity, which is the foundation of our institution. Violating that principle is one of the most serious offenses a student can commit. Community members may use this form to report academic misconduct (as listed in the Bulletin and respective policies of the schools at Columbia University) to Student Conduct and Community Standards.
Discrimination and Harassment
The University strongly encourages community members who have experienced, witnessed, or become aware of conduct that violates the Non-Discrimination Statement and Policy to submit an incident report so that the University can take appropriate steps to prevent such conduct from occurring in the future and to amend its effects. This form may be used to file a report with the University.
Gender-Based Misconduct
The University does not tolerate any form of gender-based misconduct, which includes sexual harassment, sexual assault, gender-based harassment, domestic violence, dating violence, sexual exploitation, and stalking. Community members who have experienced, witnessed or become aware of gender-based misconduct are encouraged to seek assistance, report the incident, and engage the University processes. This form may be used to file a report with the University.
General Concern
Community members may use this form to report a general concern to the University (e.g., well-being, potential policy violation, disruptive behaviors, etc.).
Other University Policy Violations
Student Conduct and Community Standards oversees resolutions to violations of other University policies where applicable. Any violations of policies set forth by other offices, Schools, or University organizations can be reported using this form.
Rules of University Conduct
The Rules of University Conduct (“Rules”) govern specifically and exclusively demonstrations, including protests, rallies, or picketing. This form may be used by members of the community to report a violation of the Rules to the University.
Undergraduate Student Life
This form is used only by CC/SEAS Undergraduate Student Life staff. Undergraduate Student Life staff includes Residential Life staff, Student Engagement staff, and Multicultural Affairs staff.
